Gastric Mucus and Pepsin Activity of Gastric Juice in Peptic Ulcer Patients


Abstract
Mucus and bicarbonate play a primary role to protect the autodigestion of gastric mucosa by acid and pepsin. To prevent the damage of gastric mucosa the mucosal barrier requires a balance between the amount of mucus secreted from mucus secreting cells and the extent of mucus degraded by proteolytic enzymes.
We had undertaken this study to determine the activities of pepsin and to evaluate the relationship between pepsin activity and soluble mucus. The results were as follows.
1) Protein content and alcian bule binding capacity of gastric juice in patients of gastric and duodenal ulcer was higher than control group (p<0.01, p<0.05)but alcian blue binding to protein ratio was not significant in control and peptic ulcer patients.
2) Free NANA, bound NANA, total NANA of gastric and duodenal ulcer groups were significantly lower than control group. And in gastric ulcer group the bound NANA was significantly decreased compared with duodenal ulcer group (p<0.05).
3) The free NANA to alcian bule binding capacity ratio was significantly lower in gastric and duodenal ulcer group compared to control group.
4) The pepsin activity of control and peptic ulcer group showed no significant difference. And the correlation between pepsin activity and alcian blue capacity or NANA revealed no clinical significance. In conclusion the alcian blue binbing capacity and free NANA content of gastric juice may have a parameter to reelect changes in mucus composition in gastric and duodenal ulcer patients but we can not find any relationship between pepsin activity of gastric juice and soluble mucus.
